POLICY FOR MANAGEMENT OF PERSONAL INFORMATION
This document describes the policy of Connected Family Psychology for the management of the practice's clients' information. The psychological service provided is bound by the legal requirements of the Australian Privacy Principles (2014) (APP) and the Victorian Health Privacy Principles (HPP).
Client files are held in a secure cabinet which is accessible only to authorised employees. The information on each file includes personal information such as name, address, contact phone numbers and other information which is relevant to the psychological service being provided.
Purpose of holding information
The information is gathered as part of the assessment, diagnosis and treatment of the client and is seen only by the psychologist. The information is retained in order to dicument waht happens during sessions and to assist with treatment. In addition, information needs to be retained to meet the legal requirements of the APP and HPP. No personal or health information is ever provided to any overseas entity.
Requests for access to client information
Clients may request to see information about them kept on file. The psychologist may discuss the contents with them or provide a copy. All requests for access to information should be lodged with Connected Family Psychology . These requests will be responded to within 14 days and an appointment will be made if necessary for clarification purposes.
If you have a concern about the management of your personal information, please inform Dr Eda Ruschena. Upon request, you can obtain a copy of the Australian Privacy Principles, which describe your rights and how your information should be handled. Ultimately, if you wish to lodge a formal complaint about the use of, or access to, your personal information, you may do so with either the Office of the Australian Information Commissioner on 1300 363 992 or the Health Complaints Commissioner on 1300 582 113.
